East Long Ave addresses the nationally recognized SDoH (Social Determinants of Health) with individuals in the community. The Social Determinants of Health are non-medical factors influencing health outcomes and well-being. By addressing these appropriately, the health and mental health of the individuals we serve can improve significantly by empowering them. East Long Ave assists individuals living in Clearfield, Jefferson, and Clarion Counties.
